Terms of Service Agreement

1. Introduction

Welcome to LA Occupational Therapy. These terms of service govern your use of our website and services. By accessing or using our services, you agree to be bound by these terms.

2. Services

LA Occupational Therapy provides occupational therapy services including but not limited to:

• Hand Therapy
• Neurorehabilitation
• Pediatric Therapy
• Activities of Daily Living Training
• Ergonomic Assessments
• Geriatric Care

All services are provided by licensed occupational therapists in accordance with professional standards.

3. Client Responsibilities

As a client, you agree to:

• Provide accurate and complete information
• Inform us of any changes in your condition
• Follow prescribed home programs
• Attend scheduled appointments or provide adequate notice for cancellations
• Respect our staff and facilities

4. Appointment Policy

Appointments are required for all services. We request at least 24 hours notice for cancellations. Late cancellations or no-shows may be subject to fees.

5. Payment Terms

We are a private pay practice. Payment is due at the time of service unless other arrangements have been made in advance. We can provide a superbill for potential insurance reimbursement.

6. Privacy

We respect your privacy and maintain confidentiality of all personal health information in accordance with HIPAA regulations. See our Privacy Policy for more details.

7. Limitation of Liability

While we strive to provide the highest quality care, we are not liable for any indirect, incidental, or consequential damages arising from the use of our services.

8. Changes to Terms

We reserve the right to modify these terms at any time. Continued use of our services after such changes constitutes acceptance of the new terms.
